Please note:  We ask that new patients provide us with their primary care physician's most recent office visit note, any recent lab tests and reports of any cardiovascular procedures.  If you've been hospitalized recently, contact the hospital and ask them to provide us with your discharge summary and any cardiovascular procedures or lab tests performed during your hospitalization.

Established patients should notify us of any changes in medications since their last office visit. Please include name, dose and schedule.  If you've had labs not ordered by Lake Cardiology since your last visit, please provide a copy to us prior to your next office visit.  Additionally, if you've been hospitalized since your last visit, please contact the hospital and have them provide us with your discharge summary and any cardiovascular studies or labs performed while hospitalized, prior to your next office visit.
